BACKGROUND: Leukemic myocarditis is extremely rare, with only 3 cases reported in the literature. Furthermore, it may be mistaken for lupus myocarditis. CASE SUMMARY: We present the case of a 16-year-old Moroccan female presenting with acute decompensated heart failure with severe biventricular dysfunction due to myocarditis with positive autoantibodies enhancing the probability of systemic lupus erythematosus diagnosis. The patient was treated with high-dose corticosteroids and biventricular function improved. However, several months later, she was readmitted with severe biventricular dysfunction and pancytopenia, and the diagnosis of acute myeloid leukemia was confirmed. Unfortunately, the patient died due to septic shock.
