Objective:To investigate the clinical characteristics of non-pregnant young patients with papillary thyroid carcinoma(PTC) across different age subgroups, so as to provide evidence for precise clinical treatment. Methods:A retrospective analysis was performed on the clinical data of 186 non-pregnant patients aged 18-35 years who were diagnosed with classical PTC by routine postoperative pathological examination in the Department of Head and Neck Surgery, Hubei Cancer Hospital from January, 2020 to January, 2026. Patients were divided into two age groups: 18-25 y(n=106) and 26-35 y(n=80). Differences in clinical characteristics between the two groups were analyzed, including maximum tumor diameter, multifocality, thyroid capsular invasion, extrathyroidal extension, coexisting Hashimoto's thyroiditis, cervical lymph node metastasis, preoperative serum thyroid-stimulating hormone(TSH), preoperative serum thyroglobulin(TG), and body mass index(BMI). The impacts of these characteristics on central lymph node metastasis(CLNM) in this population were also evaluated. Results:The 18-25 y group showed significantly higher proportions than the 26-35 y group in maximum tumor diameter>1 cm, capsular invasion rate, CLNM, number of metastatic lymph nodes>2, and BMI(P<0.01, P<0.01, P<0.05, P<0.01, P<0.05, respectively). No significant differences were observed between the two groups in multifocality, extrathyroidal extension, lateral cervical lymph node metastasis, preoperative TSH, or preoperative TG(all P>0.05). For risk factors of CLNM, maximum tumor diameter>1 cm, extrathyroidal extension, lateral cervical lymph node metastasis, high preoperative TSH, and high preoperative TG were significantly associated with CLNM(P<0.01, P<0.01, P<0.05, P<0.05, P<0.01, respectively). Multifocality, capsular invasion, coexisting Hashimoto's thyroiditis, and BMI showed no significant effects on CLNM(all P>0.05). Multivariate analysis of CLNM revealed that elevated preoperative serum TG levels may serve as an independent risk factor in this population. Conclusion:Among non-pregnant young PTC patients, younger patients tend to have larger tumor diameter, higher rates of capsular invasion and higher BMI, but are less likely to have coexisting Hashimoto's thyroiditis. Elevated preoperative serum TG levels may serve as an independent significant risk factor for CLNM. This study provides clinical evidence for preoperative treatment planning and individualized postoperative follow-up in non-pregnant young PTC patients.
