OBJECTIVE: To compare two organ-sparing laparoscopic techniques for non-parasitic splenic cysts in children (cyst fenestration and bed treatment with chemical sclerosants and bipolar cauterization; cyst fenestration and bed treatment with argon plasma coagulation). MATERIAL AND METHODS: A single-center, retrospective, comparative study included 110 children with non-parasitic splenic cysts who underwent surgery between 2010 and 2023. Patients were divided into two groups: cyst fenestration and bed treatment with chemical sclerosants and bipolar cauterization (n=29); cyst fenestration and bed treatment with argon plasma coagulation (n=81). Treatment outcomes, surgery time and hospital-stay were compared. Statistical analysis was performed using conventional statistical methods and CatBoost machine learning model. RESULTS: Recurrence rate was significantly lower in the 2nd group (2.5% (2/81) versus 34.5% (10/29), p<0.0001). Argon plasma coagulation decreased relative risk of recurrence by 13.8 times. CONCLUSION: Laparoscopic fenestration with argon plasma coagulation is a significantly more effective treatment for non-parasitic splenic cysts in children.
