AIM: This study investigated associations between SNPs in miRNA-related genes (DROSHA, DGCR8, AGO1, TNRC6B) and acute lymphoblastic leukemia (ALL) susceptibility in Chinese children and adolescents. METHODS: A case-control study of 360 patients and 398 matched controls was conducted. Genotyping used the MassARRAY platform, and multifactor dimensionality reduction (MDR) assessed potential SNP-SNP interactions. RESULTS: The DGCR8 rs1640299 was associated with an increased risk under the additive model (AOR = 1.31, 95% CI: 1.05‒1.65, p = 0.02). In males, DGCR8 rs1640299 increased ALL risk (GG vs. TT: AOR = 3.25, 95% CI: 1.49‒7.06, p = 0.003); In females, the C allele of DROSHA rs10035440 demonstrated a protective effect (TC/CC vs. TT: AOR = 0.37, 95% CI: 0.19-0.72, p = 0.004). MDR analysis suggested a high-risk four-SNP combination (OR =2.33, 95% CI:1.73‒3.14, p < 0.0001). CONCLUSIONS: This study suggests that DROSHA rs10035440, DGCR8 rs1640299, AGO1 rs636832, and rs139919 May be associated with ALL susceptibility in Chinese children and adolescents. It also suggests that potential interactions may exist between these SNPs.
