BACKGROUND: Children with leukemia undergoing chemotherapy commonly experience extreme levels of fatigue and anxiety, which can significantly impact their well-being and quality of life. Non-pharmacological nursing interventions are essential to aid manage these distressful symptoms. AIM: This study aimed to investigate the effect of Jacobson's Progressive Muscle Relaxation technique on anxiety and fatigue among children with leukemia undergoing chemotherapy. DESIGN: Randomized, controlled trial with two parallel groups was used. METHODS: Eighty children with leukemia undergoing chemotherapy were randomly assigned to two groups. Children of the control group received conventional treatment during chemotherapy sessions. While study group; children who experienced Jacobson's Progressive-Muscle Relaxation technique, beside to the conventional treatment for five consecutive chemotherapy sessions. Validated scales were used to assess anxiety and fatigue levels before and after the intervention period. SETTING: Hematology/leukemia Units at (blind for review) Children's Hospital over eleven months extending from September 2024 to July 2025. RESULTS: The study findings revealed a statistically significant decrease in both mean total score of overall and sub-scales of anxiety and fatigue in the study group compared to the control group (p < 0.05). These findings suggest that Progressive Muscle Relaxation is a potent intervention capable of producing clinically significant effect on reducing fatigue and anxiety symptoms associated with chemotherapy administration. CONCLUSION: Progressive Muscle Relaxation is a non-pharmacological, mind-body intervention that can be a valuable instrument for pediatric oncology nurses to manage fatigue and anxiety among children undergoing chemotherapy. PRACTICE IMPLICATIONS: This study directs pediatric oncology nurses to apply progressive muscle relaxation therapy for children undergoing chemotherapy to overcome the associated side effects of treatments. By systematically tensing and relaxing different muscle groups, older children can learn to recognize and release physical tension, which in turn can lead to a reduction in psychological disorders.
